✨ About The Role
- Responsible for assessing and coordinating care for assigned home health care patients and families
- Maintains patient and staff privacy and confidentiality in accordance with HIPAA regulations
- Develops and evaluates individualized plans of treatment for patients based on their physical, psychosocial, and environmental needs
- Communicates effectively with the clinical supervisor, physician, and other team members regarding patient care issues and changes in health condition
- Collaborates with the interdisciplinary team to ensure the delivery of unparalleled care to patients in their preferred home setting
âš¡ Requirements
- Experienced Registered Nurse with a minimum of 5-8 years in a clinical care setting, preferably in home health or hospice
- Strong commitment to providing high-quality care and making a positive impact on patients' lives
- Ability to work collaboratively in a team environment and lead a team of healthcare providers
- Holds a current, unrestricted state RN license and has a dependable vehicle for travel
- Comfortable with assessing patient needs, developing care plans, and communicating effectively with the clinical team
