✨ About The Role
- Provide telephonic clinical advice to members, triage clinical concerns, and connect members with necessary care resources
- Collaborate with PCP partners and Devoted Medical Group to coordinate care and deliver evidence-based healthcare
- Educate members on their conditions, explain medical terms in plain language, and teach them about potential red flags
- Work to improve operational processes, software tools, and data capabilities to enhance transitions of care case management
- Engage with members via telephonic and/or video communication to provide clinical advice and ensure they receive the care they need
âš¡ Requirements
- Experienced RN with a minimum of 4 years in ER or ICU settings, ready to provide telephonic clinical advice and triage for members
- Strong clinical skills and ability to articulate complex medical information in a clear and understandable manner to patients
- Comfortable working in a dynamic, startup environment and utilizing technology to deliver high-quality care
- Enthusiastic about making a positive impact in healthcare and dedicated to improving patient outcomes
- Willing to work remotely and able to adapt to a rotating shift schedule, including overnight shifts
