- This position involves providing Medication Therapy Management (MTM) services directly to patients through phone and video consultations.
- The pharmacist will assess patients' medications for effectiveness, safety, and adherence, incorporating lab data when available.
- Responsibilities include preparing medication summaries and personal care plans with recommendations for patients to share with their medical providers.
- The pharmacist will follow up with patients to monitor drug therapy and ensure positive health outcomes.
- The role requires collaboration with multidisciplinary teams and case managers to enhance patient care.
- A 90-day training period is required, with initial training held onsite in Phoenix, Arizona.
- After training, the position is fully remote, allowing for flexibility in work location.

- The ideal candidate will possess an Arizona Pharmacist License in good standing and must reside in the State of Arizona.
