✨ About The Role

- The Clinical Case Manager, RN will be responsible for care management and coordination activities for a Medicaid population in Dade and Monroe Counties, FL. - This role involves assessing, planning, implementing, coordinating, monitoring, and evaluating member needs and benefit plan eligibility. - The Case Manager will create a holistic approach to assess the need for referrals to clinical resources and assist in determining functionality. - The position requires traveling 10-20% of the time to meet members face-to-face in the Miami Dade County and surrounding areas. - The role includes coaching and training junior colleagues in case management techniques and processes.

⚡ Requirements

- The ideal candidate will have a strong clinical background with at least 3 years of experience in a hospital or alternative care setting. - Bilingual skills in Creole and/or Spanish are essential for effective communication with a diverse patient population. - Candidates should possess an active RN licensure in Florida and demonstrate advanced clinical judgment and critical thinking skills. - Experience in chronic disease management and case management within an integrated model is preferred. - The successful individual will be comfortable with technology, including proficiency in MS Office Suite and navigating multiple systems.
