✨ About The Role
- The Case Manager RN will be responsible for assessing, planning, implementing, and coordinating case management activities.
- This position involves conducting face-to-face member visits and telephonic assessments.
- The role requires the application of clinical judgment to address complex health and social indicators impacting care planning.
- The case manager will work with CSED waiver eligible participants and will need to consult with supervisors to overcome barriers.
- The schedule is Monday to Friday, 8 am to 5 pm, with no nights, weekends, or holidays required.

- The ideal candidate will have a current unrestricted Registered Nurse license in West Virginia.
- A minimum of 5 years of clinical practice experience is required, along with at least 1 year of behavioral health experience.
- Strong communication skills, both verbal and written, are essential for effective member engagement.
- The candidate should possess the ability to multitask and prioritize in a fast-paced environment.
- Reliable transportation is necessary, as the role requires travel within the assigned region.
