- The Case Manager RN will be responsible for assessing, planning, implementing, and coordinating case management activities with members.
- This role involves telephonic and face-to-face interactions to evaluate the medical needs of members and facilitate their overall wellness.
- The Case Manager RN will develop proactive courses of action to address issues and enhance member outcomes.
- Assessments will consider various sources of information, including co-morbid conditions and functionality impacts.
- The position requires compliance with regulatory and company policies and procedures in case management processes.

- The ideal candidate will have a current and unrestricted RN multi-state licensure in Texas and a willingness to obtain additional state licenses.
- A minimum of 3 years of clinical practice experience as a registered nurse is required for this role.
- Candidates should possess at least 1 year of experience utilizing multiple computer systems and applications, including Microsoft Word, Excel, and Outlook.
- Strong communication and interpersonal skills are essential for engaging with members and assessing their health needs.
- The successful candidate will be able to work independently and manage their time effectively while meeting business needs.
