- The Case Manager RN will facilitate the delivery of healthcare information and benefits to determine eligibility while promoting wellness activities.
- This role involves developing and implementing health strategies and policies to ensure member wellness and timely return to work.
- The position requires a collaborative process of assessment, planning, care coordination, and advocacy for individuals and families.
- The hours for this role are Monday to Friday, from 8:00 AM to 5:00 PM in the candidate's time zone.
- Training may occur in Eastern Standard Time to align with trainers' schedules.

- The ideal candidate will have an active and unrestricted RN licensure in California and Nevada.
- A minimum of 3 years of clinical practical experience, particularly in areas such as diabetes, CHF, CKD, post-acute care, hospice, or palliative care, is essential.
- Candidates should possess at least 2 years of experience in case management, discharge planning, or home health care coordination.
- Strong analytical and problem-solving skills are crucial for success in this role.
- Effective communication, organizational, and interpersonal skills are necessary to work independently and collaboratively.
