✨ About The Role
- The Nurse Case Manager will be responsible for telephonically assessing, planning, implementing, and coordinating case management activities.
- This role focuses on members recently discharged from a hospital setting to facilitate their overall wellness.
- The position requires a holistic approach to assess the medical needs of members and develop proactive courses of action.
- The work schedule is Monday to Friday from 8:00 AM to 5:00 PM EST, with no weekends or holidays.
- The role involves utilizing clinical tools and data review to evaluate member needs and benefit plan eligibility.

- The ideal candidate will have an active and unrestricted RN Licensure in Ohio or compact licensure.
- A minimum of 2 years of clinical experience is required for this role.
- Candidates should be self-motivated, energetic, and detail-oriented with strong organizational skills.
- Proficiency in computer skills, particularly with Microsoft Office applications, is preferred.
- Experience in discharge planning will be an advantage for applicants.
