- The Nurse Case Manager will be responsible for telephonically assessing and coordinating case management activities for members.
	- This role involves developing proactive strategies to enhance both short and long-term health outcomes for members.
	- The position requires the use of clinical tools and data review to evaluate member needs and benefit plan eligibility.
	- Assessments will consider various health conditions, including co-morbidities, to address functionality and care planning.
	- The role includes consulting with supervisors and presenting cases at conferences to improve overall medical management.
                            

⚡ Requirements

- The ideal candidate will have at least 3 years of clinical practice experience as a registered nurse (RN).
	- Candidates must possess a compact RN license that is active and unrestricted in their state of residence.
	- A willingness to obtain additional state licensure as needed is essential for this role.
	- Bilingual candidates, particularly those fluent in English and Spanish, will be preferred.
	- Experience in Med/Surg, managed care, and case management will be advantageous.
