- This position involves telephonic and face-to-face assessments of members to evaluate their medical needs.
- The Nurse Case Manager will develop proactive strategies to enhance members' overall wellness and address health issues.
- The role requires up to 50% - 75% travel within specified counties in Ohio.
- The working hours are Monday to Friday from 8:00 AM to 5:00 PM, with no weekends or holidays.

- The ideal candidate will be a self-motivated and energetic individual with a strong attention to detail and organizational skills.
- A successful applicant will possess active and unrestricted RN licensure or be a Licensed Independent Social Worker (LISW).
- Candidates should be tech-savvy and comfortable using clinical tools and data for case management.
- Experience in care management or a related field is essential for effectively assessing and coordinating member care.
- Strong communication and interpersonal skills are necessary to engage with members and advocate for their needs.
