- This position involves conducting comprehensive clinical assessments and developing individualized case management plans for members.
- The Case Manager RN will be responsible for monitoring and evaluating care plans and collaborating with the Medical Director as needed.

- The Case Manager RN will advocate for patients to ensure they receive the full extent of their healthcare coverage.
- The position is 100% remote, allowing the employee to work from any location within the Eastern and Central time zones.

- The ideal candidate will have an active and unrestricted RN license in the state of New York and their state of residence.
- A minimum of 3 years of acute care experience as a registered nurse is required for this position.
- Strong decision-making skills and the ability to exercise independent judgment are essential for success in this role.
- The candidate should possess well-developed interpersonal skills to effectively communicate with patients and healthcare teams.
- Experience in case management or a related field is preferred, particularly with a focus on patient advocacy and care coordination.
