- The ideal candidate will have an active and unrestricted RN licensure in the state of Michigan.
- A minimum of 3 years of clinical practical experience is required, along with at least 2 years of experience in care management or home health care coordination.
- Strong analytical and problem-solving skills are essential for assessing and addressing complex health and social challenges faced by members.
- Excellent communication, organizational, and interpersonal skills are necessary to effectively collaborate with members, providers, and community organizations.
- The candidate should be comfortable working independently from home and possess reliable transportation for travel within Wayne and Macomb counties.
