- This position involves assessing, planning, implementing, and coordinating case management activities for members.
- Care Managers will develop proactive plans of care to enhance both short and long-term health outcomes for members.
- The role requires conducting evaluations of member needs using clinical tools and data review.
- Candidates will apply clinical judgment to address complex health and social indicators impacting care planning.
- Collaboration with supervisors and interdisciplinary team members is essential to overcome barriers in achieving member goals.
- The position requires up to 25%-50% travel within Wayne and Macomb counties.
- The work schedule is Monday to Friday from 8:00 AM to 5:00 PM, with no night, weekend, or holiday shifts.
- Opportunities to work a 4-day, 10-hour work week may be available after training.

- The ideal candidate will have an active and unrestricted RN licensure in the state of Michigan.
- A minimum of 3 years of clinical practical experience is required for this role.
- Candidates should possess at least 2 years of experience in Care Management, discharge planning, or home health care coordination.
- Strong analytical and problem-solving skills are essential for success in this position.
- Effective communication, organizational, and interpersonal skills are necessary to engage with members and stakeholders.
- The ability to work independently and confidently in a remote setting is crucial.
- Proficiency in using standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, is expected.
- Bilingual candidates, particularly those fluent in Arabic or Spanish, are preferred.
