- This position is field-based, requiring significant travel to meet members in South Cook County and surrounding areas.
- The role involves managing a full caseload of waiver members, necessitating in-person quarterly visits.
- The Case Manager RN will facilitate healthcare outcomes through care coordination, support, and education for members.
- Comprehensive evaluations of members' needs and eligibility will be conducted using care management tools.
- The position requires collaboration with case managers, supervisors, and Medical Directors to address barriers to care.

- The ideal candidate will have at least 2 years of clinical experience, demonstrating a solid foundation in nursing practices.
- An active and good standing Illinois RN license is essential for this role, ensuring compliance with state regulations.
- Candidates must possess reliable transportation and be willing to travel 50-75% of the time to meet members face-to-face.
- Experience in case management and discharge planning is preferred, indicating familiarity with healthcare coordination.
