- The role involves serving as a Community Care Case Manager, focusing on member-centric care management.
- Responsibilities include assessment, planning, facilitation, care coordination, evaluation, and advocacy for members' health needs.
- The position requires interaction with members/clients both telephonically and in person, including home visits.
- The job entails collaborating with a multidisciplinary team to maximize health outcomes for members.
- The schedule is Monday to Friday, from 8 AM to 5 PM ET, with up to 25% travel required.

- A registered nurse (RN) with experience in case management and a strong understanding of healthcare systems.
- Excellent communication skills to effectively collaborate with members, providers, and community organizations.
- Ability to assess and analyze the medical and vocational status of members/clients to develop appropriate care plans.
- Experience in managing chronic and catastrophic cases, ensuring quality and cost-effective outcomes.
- Strong organizational skills to prepare documentation and coordinate case management activities.
