- This position is field-based and requires significant travel within the assigned region, approximately 80-90% of the time. - The Case Manager RN will be responsible for conducting assessments through in-person and telephonic contact with members. - The role involves creating, monitoring, and revising care plans to address the biopsychosocial needs of members. - The Case Manager will utilize motivational interviewing techniques to enhance member engagement and assess their health needs. - Collaboration with other care partners and presenting cases at multidisciplinary conferences will be key responsibilities.
