- The Case Manager RN will work remotely, with normal working hours from Monday to Friday, 10:30 am to 7:00 pm EST, with no weekend or holiday hours required. - This role involves telephonic and face-to-face assessments, planning, implementing, and coordinating case management activities for members in a high-risk maternity program. - The Case Manager RN will develop proactive courses of action to enhance both short and long-term health outcomes for members. - The position requires collaboration with multidisciplinary teams to address barriers and achieve goals in member care. - The role emphasizes a holistic approach to assessing member needs, including referrals to clinical resources when necessary.
