- The Case Manager RN will engage in a collaborative process that includes assessment, planning, facilitation, care coordination, evaluation, and advocacy for clients' health needs.
- The role involves acting as a liaison between clients, employers, providers, and insurance companies to ensure comprehensive care.
- Responsibilities include implementing and coordinating case management activities for catastrophic cases and chronically ill members across various care settings.
- The position requires telephonic and in-person interactions with clients, which may include home visits or meetings at worksites or physician's offices.
- The Case Manager will assess clients' medical and vocational statuses and develop care plans to optimize wellness and medical outcomes.

- The ideal candidate will have a minimum of 3 years of clinical practical experience, particularly in areas such as diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care.
- Experience in case management, discharge planning, or home health care coordination for at least 2 years is essential for success in this role.
- Candidates must possess an unrestricted and active RN Compact Licensure in Ohio or a compact licensure.
- Strong communication skills are necessary, as the role involves liaising with members, families, providers, and insurance companies.
