- This position is a 100% remote RN Case Manager role, requiring candidates to work Monday through Friday during specified hours.
- The RN Case Manager will be responsible for assessing, planning, implementing, and coordinating case management activities with members to evaluate their medical needs.
- The role involves developing proactive courses of action to enhance both short and long-term health outcomes for members.
- Candidates will utilize clinical tools and data review to evaluate member needs and available benefits, collaborating with plan sponsors to refer to appropriate programs.
- The position may require less than 5% travel for on-site meetings, trainings, or to meet licensure requirements.

- The ideal candidate will have an active and unrestricted RN license in their state of residence, with a preference for those in a compact RN licensure state.
- A minimum of 3 years of acute care clinical experience as an RN is required, demonstrating a strong foundation in general medical, post-surgical, and specialty care.
- Candidates should possess at least 1 year of experience utilizing computers and navigating multiple monitors while engaging in telephonic communication.
- Strong customer service skills are essential, including the ability to proactively identify and resolve issues with sensitivity to customer needs.
- Excellent critical thinking and organizational skills are necessary to effectively assess and address complex health issues and social indicators.
