✨ About The Role
- Care Managers are responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs
- Develop proactive care plans to enhance short and long-term outcomes for members
- Collaborate with members, providers, and community organizations to address health care and social determinant needs
- Utilize clinical tools and information/data review to evaluate member needs and benefits
- Conduct assessments considering information from various sources to address all conditions impacting functionality

- Minimum of 3 years of clinical practical experience and 2 years of Care Management or home health care coordination experience
- Active and unrestricted RN licensure in Illinois
- Certified Case Manager preferred
- Ability to work independently and remotely
- Excellent analytical, problem-solving, communication, and organizational skills
