- The Care Manager role at CVS Health involves assessing, planning, implementing, and coordinating all case management activities to evaluate the medical needs of members and facilitate their overall wellness

- Utilizes clinical tools and information/data review to evaluate member needs and benefits, incorporating strategies to reduce risk factors and barriers
- Collaborates with supervisors and key stakeholders to overcome barriers in meeting goals and objectives, presenting cases at interdisciplinary case conferences
- Ensures compliance with regulatory and company policies and procedures, utilizing motivational interviewing skills to engage members and discern their health status and needs

- Experienced healthcare professional with a minimum of 3 years of clinical practical experience and 2+ years of care management, discharge planning, or home health care coordination experience
- Must possess an active and unrestricted license in the state of Pennsylvania
- Comfortable working independently from home, utilizing virtual collaboration tools and demonstrating strong analytical and problem-solving skills
- Excellent communication, organizational, and interpersonal skills are essential for effective engagement with members and stakeholders
- Proficiency in computer skills, including navigating multiple systems and standard corporate software applications, is required
