⚡ Requirements

- Experienced healthcare professional with a minimum of 3 years of clinical practical experience and 2+ years of care management, discharge planning, or home health care coordination experience - Must possess an active and unrestricted license in the state of Pennsylvania - Comfortable working independently from home, utilizing virtual collaboration tools and demonstrating strong analytical and problem-solving skills - Excellent communication, organizational, and interpersonal skills are essential for effective engagement with members and stakeholders - Proficiency in computer skills, including navigating multiple systems and standard corporate software applications, is required
