✨ About The Role

- The Case Manager RN at CVS Health will be responsible for assessing, planning, and advocating for options and services to meet individual health needs - This role involves developing and implementing case management plans, monitoring and evaluating outcomes, and involving the Medical Director as needed - The Case Manager RN will need to conduct comprehensive clinical assessments, evaluate needs, and develop flexible approaches based on member needs and benefit plans - Responsibilities include advocating for patients, promoting quality and cost-effective outcomes, and identifying and escalating quality of care issues - The position requires providing a high level of customer service, utilizing assessment techniques to determine member's health literacy, and promoting lifestyle/behavior changes for optimal health

⚡ Requirements

- Ideal for a registered nurse with at least 3 years of acute care experience, who is looking to utilize their clinical skills in a remote case management role - Requires someone with strong decision-making skills, well-developed interpersonal skills, and the ability to exercise independent judgment - The successful candidate should be able to manage multiple priorities, demonstrate effective organizational and time management skills, and work well in a team - Experience in managed care, hospital settings like ICU, CCU, Step Down, ER, or OR is preferred - A compact RN License or Multi-state licensure, as well as a Certified Case Manager (CCM) certification, would be advantageous
