- Ideal candidate should have an active MA license and a minimum of 3-5 years of clinical practical experience, preferably in areas such as diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac - Strong communication, organizational, and interpersonal skills are essential for success in this role - Ability to work independently and efficiently, including working from home, is required - Proficiency with standard corporate software applications and excellent problem-solving skills are important for this position - A Bachelor's degree or equivalent experience is required, and the applicant may be required to obtain a Bachelor's degree within 3-5 years as part of role development
