- The Community Care Case Manager will use a collaborative process to assess, plan, facilitate, and coordinate care for individuals and families to meet comprehensive health needs
- Responsibilities include evaluation, advocacy for options and services, and promoting quality, cost-effective outcomes
- The role may require occasional travel within a designated geographic area for in-person case management activities
- Preferred qualifications include a Compact RN license and additional national professional certifications, although these are not required
- The typical pay range for this role is $63,200.80 - $136,600.00 per year, with actual salary depending on factors such as experience, education, and geography

- Ideal candidate should have an active MA license and a minimum of 3-5 years of clinical practical experience, preferably in areas such as diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac
- Strong communication, organizational, and interpersonal skills are essential for success in this role
- Ability to work independently and efficiently, including working from home, is required
- Proficiency with standard corporate software applications and excellent problem-solving skills are important for this position
- A Bachelor's degree or equivalent experience is required, and the applicant may be required to obtain a Bachelor's degree within 3-5 years as part of role development
