✨ About The Role
- The RN Case Manager will be responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members
- The role involves collaborating to develop proactive courses of action to enhance short and long-term outcomes for members
- Utilizes clinical tools and information/data review to evaluate member's needs and available benefits
- Applies case management processes in compliance with regulatory and company policies and procedures
- Requires the ability to work Monday through Friday between the hours of 10:30am – 7:00pm EST for a scheduled 8-hour shift, with potential for less than 5% travel for on-site meetings, trainings, or system updates

- Experienced RN with at least 5 years of clinical experience, preferably in acute care settings
- Strong ability to assess, plan, implement, and coordinate case management activities for members
- Comfortable working remotely and independently, with excellent communication skills
- Detail-oriented individual with the ability to use clinical judgment to address complex health issues
- Must be willing to work flexible hours including weekends and holidays as needed
