✨ About The Role
- The Community Care Case Manager will use a collaborative process to assess, plan, facilitate, and coordinate care for individuals and families to meet comprehensive health needs
- Responsibilities include acting as a liaison with members, employers, providers, insurance companies, and healthcare personnel, implementing case management activities, and assessing medical and vocational status
- The role involves preparing documentation, providing educational information for best medical outcomes, and applying laws and regulations related to rehabilitation services
- The Case Manager will evaluate member needs, facilitate integrative functions, and monitor progress toward desired outcomes through assessment and evaluation
- The position requires a proactive approach to address issues presented and enhance short and long-term outcomes for members' overall wellness
âš¡ Requirements
- Registered Nurse with active state license in Illinois or any active compact license
- Minimum 3+ years clinical practical experience, preferably in areas such as diabetes, CHF, CKD, post-acute care, hospice, or palliative care
- Experience in case management, discharge planning, and/or home health care coordination
- Proficiency with standard corporate software applications and efficient computer skills
- Ability to navigate multiple systems, provide ongoing case management services, and communicate effectively with various stakeholders
