✨ About The Role
- The Nurse Care Manager will be responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs
- Develops proactive care plans to address identified issues and enhance short and long-term outcomes for members
- Utilizes clinical tools and information/data review to evaluate member needs and benefits
- Applies clinical judgment to incorporate strategies to reduce risk factors and barriers and address complex health and social indicators impacting care planning
- Conducts assessments considering information from various sources to address all conditions impacting functionality, including co-morbid and multiple diagnoses
âš¡ Requirements
- Must have active and unrestricted RN licensure in the state of residence
- 3+ years of clinical experience and 2+ years of Case Management, Discharge Planning, and/or Home Healthcare Coordination experience
- Excellent analytical and problem-solving skills are required
- Bilingual skills are desired
- Effective communication, organizational, and interpersonal skills are essential
- Ability to work independently and proficient computer skills are necessary
- Bachelor’s degree or associate degree with equivalent experience is required
- Previous experience as a Certified Case Manager is preferred
- The successful candidate should have effective motivational interviewing skills and be able to ensure maximum member engagement
- Proficiency with standard corporate software applications is a must
