- The Case Manager RN will be responsible for utilizing a collaborative process to assess, plan, facilitate, and advocate for options and services to meet individual health needs
- Tasks include conducting comprehensive clinical assessments, developing case management plans, and monitoring and evaluating outcomes
- The role involves advocating for patients, promoting quality and cost-effective outcomes, and identifying and escalating quality of care issues
- The Case Manager RN will also provide coaching, information, education, and support to empower members to make independent medical and lifestyle choices

âš¡ Requirements
- Ideal candidate will have at least 3 years of acute hospital clinical experience as an RN, with a focus on general medical, post-surgical, or ICU settings
- Strong decision-making skills and well-developed interpersonal skills are essential for success in this role
- Ability to exercise independent and sound judgment, manage multiple priorities, and demonstrate effective organizational and time management skills
- Experience with Microsoft Office tools such as PowerPoint, Excel, and Word is required
- A proactive and customer-centric approach, along with the ability to work well in a team setting, are key attributes for this position
