✨ About The Role
- The Nurse Care Manager will be responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs
- Develops proactive care plans to address identified issues and enhance short and long-term outcomes
- Utilizes clinical tools and information/data review to evaluate member needs and benefits
- Applies clinical judgment to incorporate strategies to reduce risk factors and barriers affecting care planning
- Conducts assessments considering information from various sources to address all conditions impacting functionality
âš¡ Requirements
- Must have active and unrestricted compact RN licensure in the state of residence
- 3+ years of clinical experience and 2+ years of Case Management, Discharge Planning, and/or Home Healthcare Coordination experience
- Excellent analytical and problem-solving skills are required
- Bilingual skills are desired
- Effective communication, organizational, and interpersonal skills are essential
- Ability to work independently and possess effective computer skills
- Proficiency with standard corporate software applications is necessary
- Bachelor’s degree or minimum of an Associate’s degree with equivalent experience is required
