- The Case Manager RN position at CVS Health involves assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs and facilitate overall wellness - The role requires developing proactive care plans to address identified issues and enhance short and long-term outcomes - Utilizing clinical judgment to incorporate strategies designed to reduce risk factors and barriers impacting care planning - Collaborating with supervisors and key stakeholders in the member’s healthcare to overcome barriers in meeting goals and objectives - Conducting assessments that consider information from various sources to address all conditions impacting functionality

⚡ Requirements

- Minimum 3+ years of clinical practical experience and 2+ years of case management, discharge planning, or home health care coordination experience - Must have active and unrestricted RN licensure and/or a compact license - Bilingual and Certified Case Manager qualifications are preferred - Associates Degree Required, Bachelor’s Degree Preferred - Experience in developing proactive care plans, conducting assessments, and utilizing clinical tools - Ability to collaborate with interdisciplinary team members and overcome barriers in meeting goals - Strong motivational interviewing skills to ensure maximum member engagement - Experience in utilizing case management processes in compliance with regulatory and company policies and procedures
