- The role involves elevating patient care by serving dual eligible populations through best-in-class operating and clinical models
- Responsibilities include assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs
- The position requires developing proactive care plans to address identified issues and enhance short and long-term outcomes for members
- Collaborating with supervisors and key stakeholders in the memberβs healthcare to overcome barriers in meeting goals and objectives
- Utilizing case management processes in compliance with regulatory and company policies and procedures
β‘ Requirements
- Must have active and unrestricted RN licensure in the state of New Jersey
- Minimum 3+ years of clinical practice experience
- Willing and able to travel 25-50% of the time in Essex County, New Jersey
- Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
- Effective communications, organizational, and interpersonal skills
