✨ About The Role

- The Nurse Care Manager will be responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate their medical needs and facilitate overall wellness - Develops proactive care plans to address identified issues and enhance short and long-term outcomes for members - Utilizes clinical tools and information/data review to evaluate member's needs and benefits, applying clinical judgment to reduce risk factors and barriers - Conducts assessments considering information from various sources to address all conditions impacting functionality and makes referrals to clinical resources as needed - Utilizes case management processes in compliance with regulatory and company policies and procedures, ensuring maximum member engagement and discerning their health status and needs

⚡ Requirements

- Experienced Registered Nurse with a minimum of 3 years of clinical practical experience and 2 years of Case Management, discharge planning, and/or home health care coordination experience - Holds an active and unrestricted RN licensure in the state of Florida OR compact licensure in the state of residence - Possesses excellent analytical and problem-solving skills with effective communication, organizational, and interpersonal skills - Ability to work independently and proficient in computer skills including navigating multiple systems and software applications - Certified Case Manager and bilingual skills desired (English / Spanish OR English / Creole) would be a plus
