- The role involves assessing, planning, implementing, and coordinating case management activities for members with complex health and social challenges. - Care Managers will develop proactive plans of care to enhance both short and long-term health outcomes for members. - The position requires collaboration with members, providers, and community organizations to address the full continuum of health care and social determinant needs. - The job includes conducting assessments that consider various sources of information, such as claims, to address co-morbid conditions impacting functionality. - Utilization of motivational interviewing skills is necessary to ensure maximum member engagement and to discern their health status and needs.

⚡ Requirements

- The ideal candidate will have an active and unrestricted RN license in the state of Pennsylvania. - A minimum of 3 years of clinical practical experience is required, along with at least 2 years of case management, discharge planning, or home health care coordination experience. - Proficiency in standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, is essential. - The candidate should possess excellent analytical and problem-solving skills, as well as effective communication, organizational, and interpersonal skills. - Being bilingual in Spanish is preferred, indicating a capacity to engage with a diverse member population.
