- The ideal candidate will have an active and unrestricted RN license in the state of Pennsylvania.
- A minimum of 3 years of clinical practical experience is required, along with at least 2 years of case management, discharge planning, or home health care coordination experience.
- Proficiency in standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, is essential.
- The candidate should possess excellent analytical and problem-solving skills, as well as effective communication, organizational, and interpersonal skills.
- Being bilingual in Spanish is preferred, indicating a capacity to engage with a diverse member population.
