- The Case Manager RN will work full-time in a telework position, with hours from Monday to Friday, 8:30 AM to 5:00 PM CST. - This role involves assessing, planning, implementing, and coordinating case management activities to enhance the overall wellness of members. - The position requires developing proactive care plans to address identified health issues and improve both short and long-term outcomes for members. - Candidates will utilize clinical tools and data to evaluate member needs and benefits, applying clinical judgment to reduce risk factors and barriers. - Collaboration with supervisors and interdisciplinary team members is essential to overcome barriers and meet healthcare goals for members.

⚡ Requirements

- The ideal candidate will have a minimum of 3 years of clinical practical experience, demonstrating a solid foundation in healthcare practices. - Experience in case management, discharge planning, or home health care coordination for at least 2 years is essential for success in this role. - Candidates must possess an active and unrestricted RN licensure in Missouri or a compact license, ensuring compliance with state regulations. - Bilingual candidates, particularly those fluent in Spanish, will have an advantage in effectively communicating with diverse patient populations. - A compassionate approach to patient care and strong communication skills are critical for collaborating with members, providers, and community organizations.
