- The Nurse Case Manager will provide clinically and evidence-based patient care coordination for patients with behavioral and mental health needs.
- This role supports the delivery of safe, effective, high-quality, and efficient patient care at Children's Healthcare of Atlanta.
- Responsibilities include coordinating assessments and community-based services for patients discharged from the facility.
- The position requires working during the day on weekdays, specifically from 8:30 AM to 5:00 PM.
- The role is based at the Zalik Behavioral and Mental Health Center.

- The ideal candidate will have a Bachelor of Science in Nursing (BSN) or an associate's degree in nursing with a commitment to obtaining a BSN within 2.5 years of hire.
- A minimum of 3 years of experience in pediatric or behavioral and mental health care is required, with a total of 4,500 hours of clinical experience.
- Candidates should possess strong care coordination, case management, and discharge planning skills.
- Experience in community-based services and interdisciplinary discharge planning is preferred.
- A master's degree in a related field is considered a plus for applicants.
