- The Behavioral Mental Health Nurse Case Manager will provide clinically and evidence-based patient care coordination for patients with behavioral and mental health needs.
- The role involves supporting the delivery of safe, effective, high-quality, and efficient patient care at Children's Healthcare of Atlanta.
- Responsibilities include coordinating assessments, interdisciplinary discharge planning, and community-based service coordination for patients discharged from the facility.
- The work schedule is Tuesday to Friday from 7 AM to 5:30 PM.
- The position is based at the Arthur M. Blank Hospital.

- The ideal candidate will have at least three years of experience in nursing, particularly in pediatric or behavioral and mental health care.
- A Bachelor of Science in Nursing (BSN) is required, or an associate's degree with a commitment to obtain a BSN within 2.5 years of hire.
- Experience in care coordination, case management, and discharge planning is highly valued.
- The candidate should be capable of providing evidence-based patient care coordination for individuals with behavioral and mental health needs.
- Strong communication skills are essential for coordinating with interdisciplinary teams and community-based services.
