✨ About The Role
- Provide hands-on care to 12-15 patients per day, focusing on individualized patient care
- Delve into social determinants of health to understand their impact on patient outcomes
- Manage a diverse caseload ranging from preventative care to chronic condition management
- Participate in Humana/Senior Bridge's Tuberculosis (TB) screening program if selected

âš¡ Requirements
- Experienced NP or PA with 3-5 years of practice in geriatrics and/or primary care
- Holds an active, unrestricted NP/PA license in Indiana
- Comfortable working in a value-based care model and dedicated to improving patient outcomes
- Familiarity with the Value-Based Care model and HEDIS measures preferred
- Enthusiastic about contributing to an organization focused on enhancing patient experiences
