- Assist in developing, assessing, and coordinating holistic care management activities for quality, cost-effective healthcare outcomes
- Develop personalized service care plans for long-term care members and educate them on available services and benefits
- Coordinate between members, families, and care provider teams to ensure accessible services
- Monitor care plans, member status, and outcomes to provide recommendations based on identified needs
- Collaborate with healthcare providers and partners to ensure member needs are met

- Experience in care management activities and developing personalized service care plans for long-term care members
- Ability to assess member needs, identify resources, and recommend plans for optimal outcomes
- Strong communication skills to educate members and their families on available services and benefits
- Comfortable with up to 90% travel for field-based roles in Arizona
- Bachelor's degree and 2-4 years of related experience required
