✨ About The Role
- Responsible for coordinating and integrating medical and long term care services for members to ensure access to needed services
- Conduct assessments with members, caregivers, or providers to develop care plans
- Monitor service delivery and follow-up with members through in-person visits and telephonic contact
- Authorize and coordinate referrals for services and ensure provider services are delivered without gaps
- Collaborate with discharge planners, physicians, and other parties to ensure appropriate care plans and coordination of services
âš¡ Requirements
- Ideal candidate will have a bachelor's degree or Registered Nurse License with at least 2 years of care management experience, or a Licensed Practical Nurse with 4+ years of care management experience
- Experience in home health, discharge planning, or long term care is preferred for this role
- Must have a valid driver's license and be willing to travel within the specified areas
- Ability to work with elderly and physically disabled waiver members over 18 years old
- Strong communication skills and the ability to coordinate care plans effectively
