- Develop, assess, and coordinate holistic care management activities for high-risk populations with medical/physical health needs
- Evaluate and recommend care plans for complex members to achieve the best outcomes
- Act as a member advocate and liaison between members, families, and healthcare providers
- Monitor member status for complications and clinical changes, ensuring appropriate referrals and services
- Educate and coordinate community resources, provide service authorizations, and maintain accurate documentation

- Experienced Registered Nurse or Nurse Practitioner with 2-4 years of related experience in healthcare
- Skilled in developing personalized care plans and educating members on available services
- Ability to assess and coordinate care management activities for complex or high-risk populations
- Strong communication skills to act as a liaison between members, families, physicians, and facilities
- Detail-oriented with the ability to monitor and modify long-term care plans effectively
