✨ About The Role
- Conduct concurrent and retrospective reviews of patient medical records to evaluate the accuracy and completeness of clinical documentation
- Interact with physicians, patient care team, and ancillary departments to gather and clarify medical record documentation
- Identify opportunities for improvement to ensure understanding of documentation requirements and optimize reimbursement
- Enhance the measurement and reporting of outcomes by ensuring notes reflect the severity of the patient's illness and quality of care
âš¡ Requirements
- Minimum of 5 years of experience in clinical documentation improvement, patient care delivery, and/or medical records coding
- Strong clinical knowledge of diagnoses, severity of illness, risk of mortality, co-morbidities, and complications
- Ability to analyze and interpret medical record/clinical documentation and formulate appropriate physician queries
- Excellent collaboration skills to facilitate cooperation with various stakeholders and achieve desired outcomes
- Proactive approach in identifying issues, addressing concerns, and providing feedback to improve workflow processes
