✨ About The Role
- Develops, assesses, and facilitates complex care management activities for mental and behavioral health needs members
- Evaluates member needs, recommends care plans, and collaborates with resources for best outcomes
- Coordinates care plans and services for members with high acuity, identifying providers and community resources
- Monitors member status and outcomes, providing recommendations for care plan adjustments
- Provides education to members and families on procedures, healthcare options, and benefits
⚡ Requirements
- Requires a Master’s degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing and 2 – 4 years of related experience
- Licensed Behavioral Health Professional or RN based on state contract requirements
- Experience in developing, assessing, and facilitating complex care management activities for mental and behavioral health needs
- Ability to evaluate member needs, recommend care plans, and collaborate with resources
- Strong communication skills to provide education to members and their families on healthcare options
