✨ About The Role
- The role involves performing care management duties to assess, plan, and coordinate all aspects of medical and supporting services for select members
- Responsibilities include developing, assessing, and adjusting care plans to promote quality, cost-effective care
- Coordinating services between various medical providers and non-medical staff to meet clients' medical socio-economic needs
- Providing patient and provider education, facilitating access to community-based services, and monitoring referrals to support members' care management plans
âš¡ Requirements
- Ideal candidate will have a Bachelor's degree in Nursing, with a preference for a Bachelor's degree in Nursing
- Requires 2+ years of clinical nursing experience in a clinical, acute care, or community setting
- Knowledge of healthcare and managed care is preferred
- Must have a current state's RN license
- Experience in pediatric care is required for employees supporting Florida's Children's Medical Services
