✨ About The Role
- The role involves performing care management duties to assess, plan, and coordinate all aspects of medical and supporting services across the continuum of care for select members
- Responsibilities include developing, assessing, and adjusting care plans to promote desired outcomes and coordinating services between various healthcare providers
- The position requires actively participating in integrated team care management rounds and identifying related risk management quality concerns
- The candidate will need to enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems
- For certain locations, up to 30% travel may be required to perform home visits to members, and for specific states, home visits are mandatory
âš¡ Requirements
- Ideal candidate will have a compact RN license and experience working with patients in behavioral health settings, preferably located in the Eastern Time Zone
- Must have a Bachelor’s degree in Nursing and at least 2+ years of clinical nursing experience in a clinical, acute care, or community setting
- Knowledge of healthcare and managed care is preferred for this role
- Ability to assess, plan, and coordinate all aspects of medical and supporting services across the continuum of care for select members
- Strong communication skills to facilitate member access to community-based services and provide patient and provider education
