- The position involves providing overnight coverage for routine hospice care in patient homes primarily in Weld & Larimer County.
	- The role includes assessing patient needs and formulating care plans based on individual patient data and family input.
	- The nurse will implement care plans through direct patient care and coordination with other healthcare team members.
	- Evaluating patient progress and revising care plans as necessary is a key responsibility of this position.
	- The job requires working a full-time schedule of 8:00pm to 8:00am, with a 7 nights on - 7 nights off rotation.
