- This position involves assessing, planning, implementing, evaluating, and documenting nursing care of patients in accordance with organizational policies. - The role requires formulating a plan of care that includes discharge planning, utilizing assessment data and input from patients, families, and the health team. - The nurse will implement the plan of care through direct patient care and coordination of the healthcare team’s activities. - Evaluation of the patient's and family's response to care and teaching is a critical component of this role, with necessary revisions to the care plan as needed. - Documentation of all nursing interventions and patient responses must be timely and in accordance with policy, reflecting both objective and subjective data.
