The RN Navigator assesses, addresses and prevents barriers to care throughout the complex healthcare continuum. This in-person role involves face-to-face conversations with Infectious Disease patients including, but not limited to OPAT, HIV populations. This role collaborates with numerous multidisciplinary teams. This is a highly collaborative and autonomous role, where great interpersonal and communication skills are of the utmost importance. Ideal candidate: BLS, certification required. Preferred qualifications: BSN or equivalent. Experience: Clinical knowledge regarding specialty care services: Infectious Disease including, but not limited to OPAT, HIV management experience, navigation or complex care coordination/case management, clinic/ambulatory experience is helpful, extensive patient education, process improvement/development, self-driven and professional. Excellent organizational skills and clinical knowledge regarding specialty care services, as well as care coordination preferred. No new grads at this time; due to this being an experienced, higher level and autonomous nursing role.
This is a full time (40 hours/week), in-person/face to face salaried, day shift position where great interpersonal and communication skills and the ability to work autonomously are of the utmost importance. No weekends or holidays. The average day starts at 0800 and ends at 1600 and is Monday through Friday. With this role, hours are dependent on patient needs and can start earlier or later on occasion. We believe in a work-life balance and will adjust accordingly if the need arises.
