- The position involves providing comprehensive care coordination and supporting a holistic approach across the continuum of care
- Responsibilities include assessing patient's plan of care, developing, implementing, and monitoring the utilization of resources
- Collaborating with a multi-disciplinary team to facilitate timely screening, diagnosis, treatment, and increased supportive care throughout the patient experience
- Providing individual assistance to patients, families, and caregivers to help identify and overcome barriers hindering quality medical and psychosocial patient care
- Involvement in evaluating medical necessity and appropriateness of care, optimizing patient outcomes, and serving as a liaison for referring physicians

- Requires a Bachelor's degree in nursing or a related field, along with a current RN licensure in the state of practice
- Must have at least 5 years of clinical experience and excellent organizational skills
- Effective communication and writing skills are essential for this role
- Knowledge of care coordination, legal and financial aspects of diagnostic services, and health services in the specialty area is required
- Ability to teach both clinical and non-clinical personnel and understanding of process improvement are important
