As an RN Early Navigation in Medical Oncology, you will play a vital role in guiding patients through their care journey by providing comprehensive coordination in the pre-appointment phase. You'll work closely with a multi-disciplinary team to identify eligible candidates for the Multi-Disciplinary Clinic, proactively addressing barriers to care and ensuring a seamless experience prior to initial consultation. Your efforts will directly contribute to optimizing treatment outcomes and enhancing the overall patient experience in a collaborative, patient-centered environment.
Schedule: 4 days per week. 10-hour shifts between Monday - Friday. 8am-6:30pm. (No Weekends, holidays or call)

POSITION SUMMARY
This position provides comprehensive care coordination and/or assistance to patients in the pre-appointment space including, responsibilities in identifying patients that are eligible candidates for the Multi-Disciplinary Clinic. This position collaborates with a multi-disciplinary team to ensure that barriers to care are minimized prior to initial consultation, and to optimize patient treatment outcomes.
CORE FUNCTIONS
1. Assesses and addresses barriers to care through patient assessments in the pre-appointment space through direct conversation with patients and/or review of their medical records. Provides appropriate resources to patients to assist with lessening barriers found to ensure patients can attend their initial consultation. Ensures Appointment Kept Rates are above national average benchmarks.
2. Assesses medical records to determine if patient was appropriately scheduled. Documents thoroughly in the patient's medical chart findings of early navigation assessments, rescheduled appointment needs and resources provided to the patient.
3. Collaborates with clinic Social Workers, RN Navigators, and/or Patient Access Services functions for patients with complex care needs. Provides support and education to the community physician practices and prospective patients about the referral process. Serves as a resource to patients and other staff members and provides education as needed.
4. Serves as a direct contact to the patient prior to initial consultation to answer questions and concerns. Responds to patient requests for information regarding the initial consultation process and answers general questions about the facility, physician and/or available services and resources.
5. Supports process improvement activities for populations of patients to achieve the optimal clinical, financial, operational, and satisfaction outcomes. Participates in staff development to maintain current standards of practice and ensure the highest quality of care.
6. Identifies patients that are appropriate candidates for the multi-disciplinary clinics (MDC) and contacts identified patient to offer service and educate them on Multi-Disciplinary Clinic (MDC) standard operating procedures.
MINIMUM QUALIFICATIONS
Must possess a current, valid RN license in state of practice, temporary RN license in state of practice, or compact RN licensure for current state of practice. Additional certification or continuing education may be required based on area of practice.
Requires knowledge as typically obtained through a minimum of three years of current experience in specialty area. Requires excellent organizational skills, effective communication and writing skills, good time management skills and knowledge of word processing and database software applications. Also requires a good understanding of process improvement.
PREFERRED QUALIFICATIONS
Bachelor's degree in nursing is preferred.
Prior navigation experience preferred.
Oncology Navigation Certification preferred.
Additional related education and/or experience preferred.
