- This part-time position involves working 20 hours per week, specifically Monday through Thursday from 9:30 am to 2:30 pm.
- The Registered Nurse Case Manager will evaluate patients for social determinants of health and provide resources to promote healthcare compliance and wellness.
- Responsibilities include managing individual patients across the healthcare continuum to achieve optimal clinical and operational outcomes.
- The role requires establishing collaborative relationships with physicians, payers, and other healthcare team members to facilitate patient care.
- The position also involves developing and managing interdisciplinary patient discharge plans and addressing issues that may delay patient discharge.

- The ideal candidate will possess a bachelor's degree in case management or healthcare, along with a current Registered Nurse (R.N.) license in the state of Nevada.
