- This position involves assessing, planning, implementing, and evaluating nursing care for patients in their home environment. - The registered nurse will work autonomously while also having access to a supportive team structure. - The role includes formulating and revising care plans based on patient needs and collaborating with multidisciplinary teams. - The nurse will be responsible for documenting all aspects of patient care in an electronic medical record system. - The position offers a flexible schedule with options for 5/8 or 4/10 shifts and includes a weekend rotation every 4-5 weeks.
